The Chronic Disease Management (CDM) program has been put in place by Medicare to allow individuals with chronic conditions to seek out care they need. We take pride in working with our CDM patients, offering them a well-equipped and knowledgeable team to work through their complete recovery pathway, including physiotherapy, exercise physiology, and therapeutic exercise rehabilitation.

We prioritise patient care and work together with each patient’s external healthcare team including Doctor, Podiatrist, Dietician, Specialist Orthopaedic and Psychologist etc.
