The Chronic Disease Management (CDM) program has been put in place by Medicare to allow individuals with chronic conditions to seek out the care they need. We take pride in working with our CDM patients, providing them with an experienced team to help them through every step of their recovery pathway, including physiotherapy, exercise physiology, and therapeutic exercise rehabilitation.

At all phases of the recovery pathway, our team at Mead Physio and Exercise Right are qualified and well-equipped to provide clinical settings.
We place a high priority on patient treatment and are aware of how crucial it is to collaborate with other local healthcare providers in order to provide patients with a full healthcare team. We work with the external team that each patient has, which may consist of a physician, podiatrist, dietician, orthopaedic expert, psychologist, and others.

Our local, purpose-built rehabilitation facility is something we have developed over recent years and are proud of the positive impact it is having on the hills community. Exercise Right and Rehab is led by fully accredited Physiotherapists and Exercise Physiologists incorporating individual access, group exercise classes and at home programs to assist with keeping patients active and well.
Within this facility, patients with a range of medical conditions can commence personalised movement programs that are professionally supervised, cost-effective, and patient focussed.
